ABSTRACT Heart failure is one of the leading causes of mortality, both globally and in New Zealand. It is defined as the inability of the heart to meet the bodies metabolic need for oxygen and is characterised by a decrease in cardiac output. The body has many intrinsic mechanisms to attempt to maintain cardiac output, including activating the renin-angiotensin-aldosterone system (RAAS). The RAAS cascade acts to restore cardiac output by increasing fluid retention, thus increasing blood volume and pressure. Unfortunately, in decompensated heart failure, this is not enough to re-establish cardiac output, causing the action of this system to be upregulated and blood volume to increase further. This is detrimental to the already failing heart. A new drug, aliskiren, affects the pathophysiology of hypertension and heart failure by directly inhibiting renin, a mechanism that is distinct from current therapeutic agents that also target the RAAS, such as ACE inhibitors and Angiotensin II Receptor Bl ockers. This gives potential benefits by blocking the RAAS further upstream. However, clinical trials have failed to demonstrate the predicted benefits of aliskiren. Taking this into consideration, there is a strong possibility for development of further direct renin inhibiting agents displaying a higher potential therapeutic index for the treatment of hypertension and heart failure. They are also working together with each other to put together a support plan for Mr. Ali. The three key professionals are; a community psychiatric nurse, support worker and dietician. Each has a different role in the planning support for Mr. Ali as they are all supporting Mr. Ali with different aspects of his health and well-being. A Community Psychiatric Nurse is fully trained and has many years experience in a hospital setting before going out into the community. CPN’s are normally the patient’s first point of contact as they are there to support people who are experiencing a difficult time in their lives. A CPN administers medication to patients and ensure they know the reason to why they are taking then and when they should take them. A CPN does not only support the patient but also family, friends and carers. This role is valuable as it helps them to understand and cope with any illnesses the patient has. CPN’s also take referrals from GP’s, psychiatrists and inpatient wards as the CPN will support the patient and help them get back out into the community and from then on the CPN is usually the patients key-worker. This involves, getting feedback from Mr. Ali about how positive or negative the help and support is that he is receiving from the mental health team. The key-worker can be anyone who is a member of the community mental health team; a psychiatrist, CPN, Occupational Therapist, Social Worker or psychologist. A support worker is someone who supports people with different need in their everyday lives. They support them in many ways and in many situations. The STR will give Mr. Ali all the support he need with daily living whilst facilitating Mr. Ali to live an ordinary life. If Mr. Ali was to start having a relapse his STR would help identify the early signs and report it to other health professionals involved in Mr. Ali’s care and support. When Mr. Ali attends the day centre, his support worker may be with him at times to provide emotional support and could also act as an advocate. They also use a social marketing approach to inform the public about nutrition and offer unbiased advice. Public schools are the building blocks of our societies. They can be considered our foundational instruments. Although these institutes of learning play such an important role, they are unable to provide the best they can, due to their numerous flaws. It is evident that these problems exist by the number of school dropouts, falling test scores, and increased number of reported crimes. These problems all intermingle, with one another. Some can even be direct causes of each other. In other words, by making the necessary adjustments, for at least one of the problems, every problem could benefit. With the help of some basic tactics and methods, reform of public schools is possible. A basic example of this can be seen dealing with the problem of overcrowded schools. When there are too many students in a class, everyone suffers. Teachers have a hard enough time, trying to keep an average size class focused. Students are also affected, because teachers are not able to spend enough time with them individually. The overcrowding is not only a direct effect of the world's growing population, but is also caused by the lack of federal spending. To build a new school, expenses would be incurred for the construction, engineering, and furnishing of the school. To boot, infrastructure reconstruction, such as streets, sewers, and utilities, can be costly. A solution to all this is for schools to be open all year. Year-round-education (YRE) is one of the simplest and most cost-effective ways to deal with the insufficient classroom space. A school on a year-round calendar, has students in class for approximately 242 days, each year, (Brekke, 1992). In this type of schooling, instead of having on long vacation, students would have several smaller breaks. YRE is the best way to educate children, without having to cut corners. Obviously, it costs more to keep a school open for 242 days, rather than 180. All faculty, custodians, nurses, and counselors, must be on-hand, for the whole year. In actuality, it is less expensive to keep a school open, for the whole year. When you look at the per-pupil-cost, there are great savings (Brekke, 1992). Besides being cost-effective, there are multiple advantages in having kids attending school all year. Children will not forget so much information, over short breaks, compared to what they would forget over long vacations. Students would also be able to advance quicker, because teachers would not have to waste time reviewing. This is especially true, for those students whom English is a second language. A majority of the year-round schools have students attending during different periods of the year. Three groups of children would be in school, while one group would be on vacation ( ). This would allow for even more students to attend the same school. A school that would normally hold 1000 pupils would now be able to house 1300. The creation of charter schools, would be the next best idea, to handle overcrowding. During the 1960's, educators began looking for new ways to deal with the problem of overcrowded schools. Mostly in-part to court-ordered desegregation laws, school systems began opening up specialty schools. Whether a child was learning disabled or was considered at-risk, schools to help their particular needs emerged. By 1988, the American Federation of Teachers (AFT) endorsed the charter concept (Federal, 1997). Charter schools are extremely similar to public schools. These schools are free to the public. There is no discrimination in the selection process. All local, state, and federal laws must be followed. The same standardized tests, which are given to public school students, are given to charter school students. There are also various key concepts missing from public school education that are found in charter schools. One of the key concepts is charter schools' accountability. Charter schools have to show results. If only a small percentage of students graduate, a charter school could faced being closed. These schools have their charters renewed, every few years. This is a great incentive to provide the best education possible. Choice and flexibility are key factors, which make charter schools attractive. School officials can hire quality faculty members, maintain high academic standards, and allow parent involvement, because of the school's freedom. Fast Facts: Atlatl The atlatl or spearthrower is a hunting technology which was invented at least 17,000 years ago by Upper Paleolithic humans in Europe.  Atlatls give additional velocity and thrust compared to spear-throwing, and they allow the hunter to stand farther away from the prey.  They are called atlatls, because thats what the Aztecs were calling them when the Spanish arrived. Unfortunately for the Spanish, the Europeans had forgotten how to use them. The American scientific name for the spearthrower is from the Aztec language, Nahuatl. The atlatl was recorded by Spanish conquistadors when they arrived in Mexico and discovered that the Aztec people had a stone weapon that could pierce metal armor. The term was first noted by the American anthropologist Zelia Nuttall [1857–1933], who wrote about Mesoamerican atlatls in 1891, based on drawn images and three surviving examples. Carl Ann Purcell / Getty Images An atlatl is a slightly curved piece of wood, ivory, or bone, measuring between 5 and 24 inches (13–61 centimeters) long and between 1–3 in (2–7 cm) wide. One end is hooked, and the hook fits into the nock end of a separate spear shaft, itself between 3 to 8 feet (1–2.5 meters) in length. The working end of the shaft may simply be sharpened  or be modified to include a pointed projectile point. Atlatls are often decorated or painted- the oldest ones we have are elaborately carved. In some American cases, banner stones, rocks carved into a bow-tie shape with a hole in the middle, were used on the spear shaft. Scholars have been unable to find that adding the weight of a banner stone does anything to the velocity or thrust of the operation. They have theorized that banner stones may have been thought to act as a flywheel, stabilizing the motion of the spear throwing, or that it was not used during the throw at all, but rather to balance the spear when the atlatl was at rest. How To... The motion used by the thrower is similar to that of an overhand baseball pitcher. The thrower holds the atlatl handle in the palm of her hand and pinches the dart shaft with her fingers. Balancing both behind her ear, she pauses, pointing with her opposite hand toward the target; and then, with a movement as if she were pitching a ball, she flings the shaft forward allowing it to slip out of her fingers as it flies towards the target. The atlatl stays level and the dart on target throughout the motion. As with baseball, the snap of the wrist at the end imparts much of the velocity, and the longer the atlatl, the longer the distance (although there is an upper limit). The speed of a properly flung 5 ft (1.5 m) spear equipped with a 1 ft (30 cm) atlatl is about 60 miles (80 kilometers) per hour; one researcher reported that he put an atlatl dart through his garage door on his first attempt. The maximum speed achieved by an experienced atlatlist is 35 meters per second or 78 mph. The technology of an atlatl is that of a lever, or rather a system of levers, which together combine and increase the force of the human overhand throw. The flipping motion of the throwers elbow and shoulder in effect adds a joint to the thrower’s arm. The proper use of the atlatl makes spear-assisted hunting an efficiently targeted and deadly experience. Earliest Atlatls The earliest secure information concerning atlatls comes from several caves in France dated to the Upper Paleolithic. Early atlatls in France are works of art, such as the fabulous example known as le faon aux oiseaux (Fawn with Birds), a 20 in (52 cm) long carved piece of reindeer bone decorated with a carved ibex and birds. This atlatl was recovered from the cave site of La Mas d’Azil, and was made between 15,300 and 13,300 years ago. Print Collector/Getty Images / Getty Images A 19 in (50 cm) long atlatl, found in the La Madeleine site in the Dordogne valley of France, has a handle carved as a hyena effigy; it was made about 13,000 years ago. The Canecaude cave site deposits dated to about 14,200 years ago contained a small atlatl (8 cm, or 3 in) carved in the shape of a mammoth. The very earliest atlatl found to date is a simple antler hook dated to the Solutrean period (about 17,500 years ago), recovered from the site of Combe Sauniere. Atlatls are necessarily carved from organic material, wood or bone, and so the technology may be much older than 17,000 years ago. The stone points used on a thrust or hand-thrown spear are larger and heavier than those used on an atlatl, but thats a relative measure and a sharpened end will work as well. Simply put, archaeologists do not know how old the technology is. Modern Atlatl Use The atlatl has lots of fans today. The World Atlatl Association sponsors the International Standard Accuracy Contest (ISAC), a competition of atlatl skill held in small venues all over the world; they hold workshops so if youd like to learn how to throw with an atlatl, thats where to start. The WAA keeps a list of world champions and ranking master atlatl throwers. The competitions have also been used along with controlled experiments to gather field data concerning the effect of the different elements of the atlatl process, such as the weight and shape of the projectile point used, the length of the shaft and the atlatl. A lively discussion can be found in the archives of the journal American Antiquity about whether you can safely identify whether a particular point was used in bow and arrow versus atlatl: the results are inconclusive. If you are a dog owner, you may have even used a modern spearthrower known as the â€Å"Chuckit. Study History Archaeologists began to recognize atlatls in the late 19th century. The anthropologist and adventurer Frank Cushing [1857–1900] made replicas and may have experimented with the technology; Zelia Nuttall wrote about Mesoamerican atlatls in 1891, and anthropologist Otis T. Mason [1838–1908] looked at Arctic spear throwers and noticed that they were similar to those described by Nuttall. More recently, studies by scholars such as John Whittaker and Brigid Grund have focused on the physics of atlatl throwing, and trying to parse out why people eventually adopted the bow and arrow. The Knowledge is Power Program (hereafter referred to as KIPP) has been the recipient of much praise. In the recent documentary, Waiting for Superman, KIPP was lauded as one of a handful of educational programs for poor children of color that is making a difference in closing the achievement gap. Diane Ravitch wrote in her recent book, â€Å"[T]he charter schools with the most impressive record of success are the KIPP schools† (Ravitch, 2010, p.135). Washington Post Education Columnist, Jay Mathews wrote an entire book on the schools, calling them â€Å"the most promising schools in America.†(Mathews, 2009) So, why is KIPP garnering all of this attention and is it really narrowing the achievement gap? KIPP was started in 1994 by Teach for America alumni David Levin and Michael Fienberg, who had been working in inner-city Houston schools. KIPP was created with a singular goal in mind; to get low-income students of color to college. The data would suggest that these schools have been successful in achieving that goal. During the 2008-2009 academic year, KIPP enrolled 21,831 students, 95% of whom were African-American or Latino (KIPP Foundation, 2009). Of those students who finish 8th grade at a KIPP school, 95% graduate from high school and 88% of them matriculate to college. These compare to national rates of 70% high school graduation and 40% college matriculation for low-income students (KIPP Foundation, 2009). According to a recent study of 22 KIPP schools, within three years, half of all KIPP schools in the study closed one half or more of the black-white achievement gap in math, and one third of the black-white achievement gap in reading. In these KIPP schools, student gains are equivalent to 1.2 years of additional growth i... ...the tools to significantly close the achievement gap, but they won’t likely be responsible for a radical upward mobility of low-income students of color. So, what role can KIPP play in the larger picture? Diane Ravitch offered sage advice in a recent speech at Rice University when she challenged KIPP co-founder Mike Fienberg in saying, â€Å"Don’t compete. Collaborate with public schools. Share whatever you know that works. Work with public schools. Help them get better. We all share the same goal and that is to ensure that every child in America gets a great education† (Ravitch, 2010). Perhaps KIPP schools are best utilized as â€Å"laboratories† where methods are tested, refined and readied for export to the public school system at large and utilized by all students. No adult enjoys being fed vast amounts of theory with little or no practice. As we grow older, we much prefer a â€Å"hands-on† approach to things. The learning will be much more effective if we can take an active role in the learning process. Thus it’s important to encourage objective discussion both in analyzing the problem and coming up with a solution. 4)Adults will only learn by solving problems they can associate with their reality. They focus on â€Å"real world† problems and practical assimilation of what is being taught. 5)Experience will interfere in adult education. Thus, any new information being presented must be integrated with their own experience. 6)Adults learn better in an informal environment. They require a more relaxed atmosphere; one that will stimulate them to participate, 7)Adults need feedback. it’s important that learning process provide adults with constructive feedback, through self-evaluation questionnaires and activities. 8)Adults require a variety of teaching methods. The use of audiovisual materials is highly recommended with use of interactive activities such as role-playing, mock classes and presentations from the students.
